In Vitro Fertilization (IVF)

Assisted reproductive technologies

Treatment options. The treatment of infertility depends on its underlying cause, age, duration of attempts to conceive and the couple’s preferences. Available options range from simple medical interventions to more advanced assisted reproductive techniques.

Cycle monitoring and ovulation induction are the simplest and most fundamental approaches to infertility treatment. Follicular development is monitored through ultrasound examinations and blood tests, allowing identification of the optimal timing for intercourse and thereby increasing the likelihood of natural conception.
For women who do not ovulate regularly, oral medications that stimulate ovarian function (such as clomiphene citrate or letrozole) or low-dose injectable gonadotropins may be prescribed to promote the development of multiple follicles. This approach is generally well tolerated and can result in pregnancy without the need for more complex interventions.

Intrauterine insemination is a relatively simple assisted reproductive technique in which processed sperm from the male partner is placed directly into the uterine cavity using a thin catheter. This increases the number of sperm reaching the fallopian tubes, thereby improving the chances of fertilization and successful pregnancy.
IUI is primarily indicated in cases of male-factor infertility (e.g., low sperm count, reduced motility or ejaculatory dysfunction). The technique allows sperm to bypass two significant barriers: the acidic vaginal environment and cervical mucus.
It may also be recommended for couples who are frequently apart during the ovulatory period due to professional or social reasons. For the procedure to be appropriate, the woman should have patent fallopian tubes, a healthy endometrium, an adequate number of good-quality oocytes, and normal hormonal function.

In vitro fertilization is the most widely used assisted reproductive technique. As the name implies, fertilization occurs outside the body—in the embryology laboratory—rather than within the fallopian tube. The procedure involves retrieving oocytes from the ovaries, fertilizing them with sperm in the laboratory and transferring the resulting embryos into the uterus.

The main methods include “conventional” IVF and intracytoplasmic sperm injection (ICSI).

IVF can be performed using the genetic material of both partners or with donor oocytes or sperm. In addition, the procedure may involve the use of fresh or cryopreserved oocytes, sperm, or embryos.

Stages of in vitro fertilization (IVF)

The selection of the appropriate IVF protocol is not the same for all women. It is always individualized based on age, ovarian reserve, test results, previous treatment history and the couple’s goals.

After the evaluation is completed, it is decided whether the attempt will be carried out in a natural cycle, with mild stimulation (mini-IVF), or with full ovarian stimulation—each time choosing the method that offers the highest chances of success with the greatest safety.

  • Natural Cycle IVF

In a natural cycle, no medications are administered. The natural development of the single follicle that matures each month is monitored and when it reaches the appropriate stage, oocyte retrieval and fertilization are performed.
This approach is preferred for women who cannot or do not wish to receive hormonal treatment. It is also an option for women with very low ovarian reserve.

  • Mild Stimulation (Mini-IVF)

This method uses lower doses of medication compared to conventional protocols. The goal is to obtain a small number of high-quality oocytes for fertilization.
Mini-IVF is suitable for women with moderate ovarian reserve, in cases where reduced medication is desirable, or when preferred by the couple.

  • Conventional Ovarian Stimulation

This is a commonly used and highly effective protocol. Injectable gonadotropins are administered for 8–12 days to stimulate the maturation of multiple oocytes simultaneously. Follicular development is monitored through ultrasound scans and blood tests.

Different protocols (e.g., GnRH antagonist or GnRH agonist protocols) are selected on an individualized basis depending on the woman’s profile.

Full stimulation is the most common choice and offers the highest success rates, especially when the goal is to create multiple embryos.

When the follicle(s) reach the appropriate size, final oocyte maturation is triggered using hCG or a GnRH agonist. Oocyte retrieval is scheduled approximately 36 hours later.

Approximately 36 hours after triggering oocyte maturation, oocyte retrieval is performed. This is a painless and very safe procedure carried out under ultrasound monitoring via the vagina, usually with light sedation. The procedure lasts about 10–20 minutes, and the patient can return home the same day.
The collected oocytes are immediately transferred to the embryology laboratory, where their maturity is assessed. On the same day, a sperm sample is obtained from the partner or donor and undergoes specialized processing.
Sperm is typically collected through ejaculation; however, alternative retrieval methods may be used, particularly in cases such as obstructive azoospermia.
Today, in the majority of cases, fertilization in the embryology laboratory is performed using ICSI (Intracytoplasmic Sperm Injection). With this technique, a single sperm is injected directly into the oocyte using a very fine needle.
ICSI involves the selection of high-quality, mature spermatozoa, with lower rates of aneuploidy (abnormal chromosome number). It achieves high fertilization rates (over 60%).
Following fertilization, embryos begin to develop in the laboratory under controlled conditions.

Embryos are cultured for 3 to 5 days. On day 3, they are at the cleavage stage, while on day 5 they typically reach the blastocyst stage.

Embryo transfer can be performed either in the same cycle as oocyte retrieval and fertilization (fresh cycle) or in a subsequent cycle after cryopreservation and thawing (frozen cycle). In both cases, a well-prepared endometrium—the inner lining of the uterus where the embryo will implant—is essential.

The number of embryos transferred is determined by the physician and embryologists in consultation with the couple.

On the day of embryo transfer, the woman is prepared in designated clinic rooms. Fasting is not required; however, a full bladder is recommended to improve ultrasound visualization.

Using specialized catheters and ultrasound guidance, the physician performs the embryo transfer. The woman remains lying down for approximately 20–30 minutes and can then leave the clinic.

For the following 4 days, it is recommended to avoid strenuous physical activity. After this period, normal daily activities can be resumed.

A blood test measuring β-hCG (beta-human chorionic gonadotropin) is performed 10–14 days after embryo transfer to confirm pregnancy.

Preimplantation Genetic Testing (PGT)

Preimplantation Genetic Testing (PGT) is an advanced technique that allows the analysis of the genetic material of embryos following IVF and before embryo transfer. This ensures that only healthy embryos are selected for transfer to the uterus, reducing the risk of passing on genetic disorders.
PGT combines in vitro fertilization with specialized genetic diagnostic techniques. The procedure involves a biopsy of a few cells from the embryo at the blastocyst stage, followed by detailed laboratory analysis. The most common types of PGT include:

This test detects chromosomal abnormalities and is often recommended for women of advanced maternal age or those with a history of recurrent miscarriages.
The most common viable chromosomal abnormality is Down syndrome (Trisomy 21). Other chromosomal abnormalities are not compatible with life and often result in miscarriage during the first trimester.
The likelihood of chromosomal abnormalities increases with maternal age. In some couples, a higher proportion of embryos may carry chromosomal abnormalities, which can lead to repeated pregnancy loss.
.

This test identifies structural chromosomal abnormalities, such as translocations or inversions.

This test is used when there is a known risk of a specific inherited genetic condition. In Greece, the most common genetic disorder is β-thalassemia (Mediterranean anemia), followed by cystic fibrosis.
PGT-M can be successfully applied to any genetic disorder, provided that the specific genetic mutation responsible for the condition in the family has been identified.

PGT may:
• Increase the chances of a successful pregnancy
• Reduce the risk of miscarriage in selected cases
However, it is not necessary for all couples. The decision to proceed with PGT is made after genetic counseling and individualized medical evaluation.

Fertility Preservation

Every woman is born with a finite number of eggs—approximately 1 to 2 million. By puberty, this number declines significantly to around 300,000–400,000. Over the course of her reproductive life, only about 300–400 eggs will mature and be released, while thousands are lost each month. This decline continues over time, accelerating after the age of 35, and ultimately leads to menopause when approximately 1,000 eggs remain.
Fertility preservation is a modern medical option that allows both women and men to safeguard their reproductive potential for the future. Its purpose is to preserve eggs, sperm or embryos in situations where fertility may be compromised—such as before cancer treatments, surgical procedures, or due to age, personal, or professional choices.
Early awareness and timely decision-making are crucial, as fertility preservation is most effective when performed proactively. It offers reassurance, freedom of choice and hope for the future, empowering individuals to decide when and how to start a family.

Egg freezing is one of the most important advances in assisted reproduction, allowing women to preserve their fertility for the future. With this method, eggs are collected and frozen at very low temperatures in the embryology laboratory, so they can be used later—even many years afterward.

The process includes:
• Ovarian stimulation to promote the development of multiple follicles
• Egg retrieval, a simple and painless procedure performed under light sedation
• Assessment and freezing of mature eggs using vitrification, a method that ensures very high survival rates after thawing (>90%)

Indications for egg freezing:
• Medical reasons: Prior to treatments that may negatively affect ovarian reserve (e.g., chemotherapy or radiotherapy)
• Risk of premature ovarian insufficiency: In women with a family history or genetic predisposition
• Social reasons: When a woman chooses to delay motherhood for personal or professional reasons, preserving fertility while egg quality is optimal

Safety and Effectiveness

Modern vitrification techniques protect eggs during freezing without compromising their quality. This method prevents the formation of ice crystals, which in the past negatively affected egg viability and fertilization potential, thus contributing to higher pregnancy rates.
More than 6,000 babies have been born worldwide from frozen eggs. Egg freezing is considered safe for both the woman and the future child. Scientific studies show no increased risk of genetic abnormalities compared to embryos derived from fresh eggs, nor higher rates of pregnancy complications or preterm birth.
Age at the time of freezing is a key factor for success. Ideally, egg freezing should be performed before the age of 35, when egg quality is higher. However, it can also be considered later, provided there is sufficient ovarian reserve.
Egg freezing allows women to take control of their reproductive timeline and is a valuable tool in modern reproductive medicine—either as a preventive option or as part of a treatment plan.

Sperm freezing involves the collection and long-term storage of sperm in liquid nitrogen for future use in assisted reproductive techniques.
It is recommended for men who wish to preserve their fertility due to:
• Medical treatments (e.g., chemotherapy, surgery)
• Exposure to factors that may affect sperm quality
• Ejaculatory disorders
This method ensures that reproductive potential is safely preserved for the future.

Embryo freezing involves the rapid freezing of embryos created through IVF for future use. It is one of the most effective fertility preservation methods, with survival rates after thawing approaching 100%.
This option requires the use of sperm from a partner or donor, as fertilization must occur before freezing. In many cases, all embryos retrieved are frozen, allowing for transfer at a later, more suitable time. The process requires a short preparation period of approximately 1–2 weeks, including ovarian stimulation, egg retrieval, and fertilization.
Embryo cryopreservation offers high success rates and is a reliable option for couples wishing to secure their future reproductive potential.

Other Cutting-Edge Technologies in Assisted Reproduction

Assisted Reproductive Technologies are evolving rapidly, becoming increasingly accessible, precise, and effective, shaping the future of reproductive medicine.

The integration of “smart” systems based on artificial intelligence (AI) and machine learning is transforming fertility care.
The Future Fertility platform, through its Violet algorithm, analyzes images of mature oocytes and provides automated predictions regarding:
• The likelihood of blastocyst formation
• The probability of achieving a live birth
Violet offers both clinicians and patients valuable insights into the developmental potential of frozen eggs and can support clinical decision-making—for example, whether additional egg freezing cycles may be recommended.

Time-lapse incubators allow continuous monitoring of embryo development through sequential imaging, which is compiled into video. This technology enables embryologists to closely observe developmental patterns without removing embryos from their optimal environment, improving embryo selection while maintaining stable culture conditions.

For women with reduced ovarian response due to diminished ovarian reserve, poor oocyte quality, or approaching menopause, ovarian rejuvenation therapy using PRP offers a promising approach. This method involves isolating platelet-rich plasma (PRP), rich in growth factors, from the patient’s own blood. The PRP is then injected into the ovaries with the aim of activating cells and potentially promoting the development of new oocytes.
In addition, PRP can be applied to the endometrium to enhance vascularization and improve uterine receptivity, thereby increasing the chances of successful embryo implantation.

Donation

Egg donation offers women who are unable to conceive using their own eggs the opportunity to achieve pregnancy using donor oocytes.
Indications for this method include:
• Advanced maternal age
• Premature ovarian insufficiency (POI)
• Surgical removal of the ovaries (partially or completely)
• Congenital absence of ovaries
• History of multiple unsuccessful IVF attempts due to poor ovarian response or low egg quality

In Greece, egg donation is regulated by a strict legal framework (Law 3305/2005) and is carried out under the supervision of the National Authority of Assisted Reproduction. Donors must be under 35 years of age and have a clear medical history. They undergo thorough screening, including:
• Karyotype analysis (to detect chromosomal abnormalities)
• Ultrasound examination
• Screening for infectious diseases (such as hepatitis, HIV and syphilis)
• Genetic testing for inherited conditions (e.g., cystic fibrosis)
Greek legislation stipulates that donation is generally anonymous, aiming to protect the donor, the recipient and the child. However, identified (non-anonymous) donation is also permitted in cases of a familial relationship, for example when a woman wishes to receive eggs from a sister or cousin.

Sperm donation involves the use of donor sperm and is recommended for:
• Couples with infertility due to male factors
• Single women wishing to conceive
• Cases where there is a risk of transmitting genetic diseases
Donor sperm can be used either for intrauterine insemination (IUI) or fertilization through in vitro fertilization (IVF).
Prospective donors must have a clear medical history and undergo extensive evaluation, including semen analysis, karyotype testing, physical examination, psychological assessment, screening for infectious diseases.

Embryo donation offers infertile couples the opportunity to build a family when other fertility treatments have failed. Embryos may originate from couples undergoing fertility treatment who choose to donate their cryopreserved embryos instead of discarding them or the combination of donor sperm and donor eggs

This option is recommended when:

  • Both partners have infertility factors, reducing the likelihood of success with their own gametes
  • A woman without a partner wishes to conceive

This method has high success rates, as the eggs used typically come from young, healthy donors.

Nutrition & Fertility: The Role of Personalized Support

Fertility is influenced by a variety of factors, with nutrition being one of the most significant. A well-designed dietary approach can play a key role in the reproductive journey, affecting the chances of success in both natural conception and assisted reproductive treatments such as in vitro fertilization (IVF). A balanced and personalized nutrition plan, tailored to the individual’s needs and based on their medical and dietary history, can positively support conception by promoting reproductive health, hormonal balance and overall well-being.

Nutrition is a powerful tool in fertility management. With proper guidance, individuals and couples can enhance their chances of conception and lay strong foundations for a healthy pregnancy from the earliest stages.
For this reason, collaboration with a specialized Dietitian–Nutritionist is essential, as it allows for a personalized approach and targeted nutritional support at every stage of the reproductive process.
A tailored nutrition plan can significantly contribute to regulating hormonal balance, which is fundamental for ovulation, fertilization and embryo implantation.
At the same time, a balanced diet has been associated with improvements in egg and sperm quality, key factors in achieving successful conception.
Personalized nutrition can also help reduce inflammation in the body, which may negatively impact reproductive function and the body’s ability to support embryo implantation.
In addition, maintaining a healthy body weight through proper nutrition is particularly important for IVF success, as it supports optimal reproductive function and hormonal regulation.

Certain medical conditions such as insulin resistance or gluten intolerance, can affect fertility. A personalized nutrition plan can help manage and improve these conditions, offering comprehensive support for reproductive health.
Nutritional guidance in these cases goes beyond weight management, focusing on strengthening the overall reproductive effort.

At Prolific, personalized support is a cornerstone of success in both fertility and assisted reproduction. Every couple has unique needs and tailored nutritional guidance is essential not only for managing body weight or clinical parameters, but also for supporting the entire journey through conception attempts or IVF cycles.

Overall, nutrition is not merely supportive but it is a critical factor in enhancing fertility and maintaining a healthy pregnancy. Working with a specialized Dietitian–Nutritionist and following a personalized nutrition plan can significantly improve the chances of conception, while also promoting overall health, well-being and the success of assisted reproduction.

Psychological Support in In Vitro Fertilization (IVF)

The journey to parenthood through in vitro fertilization is an experience filled with hope, uncertainty and profound emotional changes. Throughout this process, medical and technological advances intersect with each individual’s personal story. At every stage, from the decision to begin treatment through pregnancy and into the postpartum period, new psychological challenges may arise, which are often invisible or overlooked within the clinical framework of assisted reproduction.

The goals of psychological support during this journey are to strengthen emotional resilience in the face of the emotional, physical and practical demands of the process and to help manage the uncertainty that accompanies each stage.
Support can help alleviate the psychological burden of repeated medical procedures and potential unsuccessful attempts, enhance communication and cooperation within the couple, manage social pressures and expectations and explore and process feelings of loss or traumatic experiences related to infertility.
For many women, the process may generate ambivalence toward their body, nature or sense of purpose, as well as feelings of shame, failure or anger. For men, the pressure to remain “strong” and supportive often conceals their own emotional strain, anxieties and feelings of helplessness. Psychological support provides a safe space for both partners to express their emotions, understand their needs, develop realistic expectations, build self-care skills and reconnect meaningfully with one another.

Psychological care is not limited to the treatment period but extends beyond it, supporting the transition into parenthood and the emotional adjustment regardless of the outcome. It creates a safe environment in which the IVF experience can be integrated into a broader personal narrative, one of growth, understanding and self-awareness. The goal is not only the outcome of treatment, but also the strengthening of the individual and the relationship throughout this unique journey.

With respect for each couple’s personal story, our psychological support programs are designed to offer space for understanding, emotional relief and meaningful connection within an environment of safety, trust and professional care.
